DOCX · 9 KBLitigation
Summary
An affidavit is a written statement of facts that you swear or affirm is true before a Commissioner for Oaths or a notary. This template gives you the standard layout: an optional court heading and case title, the deponent's details, the facts in numbered paragraphs, and the jurat (the part the Commissioner completes).
State only facts you know to be true, one point per paragraph, and refer to any document you attach as an exhibit. Delete the court heading if the affidavit is not for a court case. It is a general template and not legal advice.
Main provisions
- Court heading and case title (optional — delete if not for a court case).
- Deponent's full name, address and occupation.
- Numbered paragraphs for stating facts and referring to exhibits.
- Standard opening and truth-of-contents paragraphs.
- Jurat and signature block for the deponent and the Commissioner for Oaths.
